Pipeline stage status change notes

When a stage is set to "snag", it would be useful to be able to add a reason. This may come in useful at various points during the lifecycle of a job, for example:
- Aborted site visits - Surveyor was unable to access the site.
- Quality control - QC checks failed.
- Issuing reports - The report could not be issued to the client.
Another scenario could be that there were no samples taken during an Asbestos survey. When marking the 'Lab Results' as complete, it can be recorded that no samples were taken/results entered.
